Application and Fees
Performance Improvement CME Application
The Office of Continuing and Community Education strongly
encourages program planners to begin the application process
as far in advance of the CME activity as possible. In order
to receive consideration, the Office needs to receive
completed CME Application at least 8 weeks in advance of the
educational activity. The application and planning process
involves multiple steps that require significant time and
effort, as well as some interaction with the Office.
To initiate a PI CME Application, please call the Office
at 860-679-2094 or 860-679-7647. Based on a brief
discussion, you will be sent an electronic
application, including some pre-filled items. Please do not
initiate an application without this discussion. Program
planners who are initiating this process for the first time
should avail themselves of the opportunity to meet with
Office staff to review the application and its process.
